SeeYouDoc's Know Your Customer (KYC) Policy
Purpose
The purpose of this Know Your Customer (KYC) Policy is to ensure that SeeYouDoc maintains the highest standards of integrity and security while onboarding patients, healthcare providers, and medical organizations. This policy aims to prevent fraudulent activities, comply with regulatory requirements, and protect the privacy and security of all users on the SeeYouDoc platform.
Scope
This policy applies to all individuals and entities that wish to use the SeeYouDoc platform, including:
Patients
Healthcare Providers
Medical Organizations (Hospitals, Clinics, and Networks)
KYC Requirements
Patients
Patients are required to provide the following information when they submit their profile/s for verification:
Full Name
Date of Birth
Mobile Number
Address
Valid Government-Issued ID (e.g. passport, driver's license, national ID)
Proof of ID
All patient information is securely stored and used solely for identity verification and healthcare service purposes.
Healthcare Providers
Doctors who wish to register on SeeYouDoc must provide:
Full Name
Professional Credentials (PRC license, PTR and S2 licenses (if applicable))
Specialties
Proof of Specialties
For doctors with private practices:
Bank Account (for payout purposes, securely handled and verified).
SeeYouDoc thoroughly verifies the credentials and licenses of all healthcare providers to ensure that they meet the necessary standards for providing medical care.
Medical Organizations
Medical Organizations wishing to onboard their networks, facilities, or clinics to the SeeYouDoc platform must provide:
Organization Name
Organization Type
For organizations with legal identity, the following additional information are required:
Legal Name
Business Structure
Tax Identification Number (TIN)
Tax Type (VAT or Non-VAT)
Once approved, they may now add their networks, facilities, or clinics under their organization. Each addition will undergo the same KYC process before accessing the platform. All organizations must designate a primary contact person responsible for managing their account on SeeYouDoc.
Verification Process
Identity Verification: SeeYouDoc uses secure, industry-standard methods to verify the identities of all patients, doctors, and medical organizations. This may include cross-referencing information with official databases and employing third-party verification services.
Document Review: All submitted documents are reviewed by the SeeYouDoc team to ensure authenticity and validity.
Ongoing Monitoring: SeeYouDoc conducts regular audits and monitoring of user activities to detect and prevent fraudulent activities and ensure compliance with this policy.
Data Privacy and Security
All personal and organizational information collected during the KYC process is handled in accordance with SeeYouDoc's Privacy Policy and applicable data protection laws. Data is encrypted and securely stored, and access is restricted to authorized personnel only.
Compliance and Updates
SeeYouDoc is committed to complying with all applicable legal and regulatory requirements regarding customer identification and verification. This KYC policy will be reviewed and updated periodically to reflect changes in regulations and best practices.
Consequences of Non-Compliance
Patients
Patients who do not complete the KYC verification process will still be able to book appointments on the SeeYouDoc platform. However, they will have limited access to certain specialized services and programs, such as the TB program of Parañaque. This limitation is necessary to maintain the security and integrity of the platform and to ensure compliance with regulatory standards.
Healthcare Providers
Non-compliance with KYC requirements by healthcare providers or medical organizations may result in the denial of access to the SeeYouDoc platform or the unpublishing of existing accounts. SeeYouDoc reserves the right to report any suspicious activities to relevant authorities as required by law.
