How To Issue a Service Request as a Practitioner?
Step 1
While on the Encounter page, you may click the "Clinical" tab.

Step 2
Then, click "Service Request"
You may create a service request before, during, or right after the encounter has ended.

Step 3
Click "New Service Request".

Step 4
Choose whether you will assign the patient to a facility within the same organization or to a different clinic under your facility and organization.


Step 5
Select the clinic/facility where you will refer the patient.

Step 6
Select care plan (if applicable). Otherwise, you may just select N/A.
Note: The care plan is only available for SeeYouDoc's TB Module.

Step 7
Select category.

Step 8
Select priority.

Step 9
Enter note.

Step 10
Click "Save".

Step 11
By default, the service request created will not yet be visible to the patient.
Click on the service request created.

Step 12
You may preview what the service request would look like on the patient's POV by clicking the "Preview" button.


Step 13
To send the medical certificate to the patient:
Click "Finalize".

Step 14
Review service request contents and don't forget to tick the "Send to patient" checkbox.

Step 15
Click "Save".

Step 16
The service request will now be issued and available to be viewed by the patient.
