How do I refer a TB patient to facilities under my network?

Once a TB enrollment has been accepted, they will now become TB patients under your network.


Step 1


On your Network portal, navigate to Patients, and select any patient.



Step 2


Navigate to Care Plans.



Step 3


You can only create referrals on active care plans. Draft care plans must first be set to Active.



Step 4


Edit the Care Plan as needed.





Step 5


Go to the Referrals tab, and click New Referral.



Step 6


In the New Referral form, select the facility you wish to refer the patient to. Only facilities part of your network will show up as options.



You may need to contact your organization admin to onboard a missing facility.

Step 7


Complete the rest of the information needed for the request. Once ready, click Save.





Step 8


An alert will appear on the top-right corner to signify the success of the transaction. Your patient has now been referred to the facility.
